Ziektepresentatie
Ziektebeschrijving

Niveau 3 |
Long Chain 3-Hydroxyacyl-CoA Dehydrogenase Deficiency (LCHADD, MIM 609016) and Mitochondrial Trifunctional Protein (MTP) Deficiency (MIM 609015) are autosomal recessive disorders of the fatty acid oxidation.[8] |
|
Niveau 3 |
The mitochondrial trifunctional protein is an enzyme complex composed of three enzymes that catalyze the mitochondrial beta-oxidation of fatty acid: Long Chain 3-Hydroxyacyl-CoA Dehydrogenase (LCHAD), Long Chain Enoyl-CoA Hydratase (LCEH) and Long Chain Ketoacyl-CoA Thiolase (LCKAT).[9] |
|
Niveau 3 |
The HADHA gene encodes LCHAD and LCEH, whereas the HADHB gene encodes LCKAT. Both enzymes are located at chromosome 2 (2p23). Mutations in either subunit of the MTP can result in MTP deficiency with reduced activity of all three MTP enzymes. The 1528 G>C mutation causing the missense mutation E510Q in the MTP α-subunit is frequent in patients with LCHADD.[10] No clear phenotype-genotype associating exists.[9, 11] |
|
Niveau 3 |
MTP deficiency (MTPD) is characterized by decreased activity of all 3 enzymes, whereas LCHADD is caused by isolated LCHAD-enzyme deficiency.[10, 12] |
Etiologie
|
Niveau 3 |
The estimated prevalence varies from 1:250.000 (LCHADD) / 1:750.000 (MTP-deficiency) in the USA and Australia to 1:50.000 (LCHADD) in Sweden and 1:300.000 in the Netherlands.[14-16] |
Prognose
Hielprik
Afwijkend indien C16OH-carnitine ≥ 0,08 μmol/l.

Niveau 3 |
The clinical spectrum of LCHADD and MTPD is broad and can involve hypoglycemia, myopathy, retinopathy and cardiomyopathy.[1, 9, 11, 18] |
|
Niveau 3 |
As a result of the LCHADD or MTPD the long chain fatty acids cannot be oxidized. This leads to symptoms associated with energy deficiency in fatty acid oxidation-dependent tissue (heart, liver, muscle) and the toxic properties of accumulating intermediates. The symptoms can be triggered by prolonged fasting, exercise, infection, exposure to cold or fat-enriched diet.[11] Even in the fed state mitochondrial β-oxidation is an important source of energy for high energy consuming tissues, but it becomes essential when carbohydrate stores are depleted.[9] All these symptoms are reversible with sufficient supply of energy. However the severe form is still highly associated with neonatal death, since fulminant acute symptoms may be therapy-resistant and the patient may be deceased even before diagnosis.[9, 11] |
|
Niveau 4 |
Peripheral neuropathy (causing progressive muscle weakness, muscular atrophy and sensory disorders) and retinopathy (with increasing visual field defects, deterioration of color vision and dark adaptation) are irreversible long-term complications in LCHADD and MTPD.[9, 11] These are possibly due to the toxic effect of the accumulation of hydroxyl acylcarnitines.[11] Another hypothesis is that docosahexanoic acid (DHA) deficiency plays a role in pigmentary retinopathy.[19] Peripheral neuropathy is most common in MTPD (80%), whereas retinopathy is more common in LCHADD patients (30-50%).[11] Some cases of hypoparathyroidism are reported for both LCHADD and MTPD.[2, 3] |
|
Niveau 4 |
Furthermore, pregnant mothers carrying a child with LCHADD or MTPD are at risk for HELLP (Hemolysis, Elevated Liver enzymes, Low Platelets) and AFLP (Acute Fatty Liver of Pregnancy).[6, 20] |
Langetermijncomplicaties:
- Progressieve spierzwakte en atrofie bij perifere neuropathie
- Retinopathie (o.a. gezichtsvelduitval).

Niveau 3 |
The main purpose of the therapeutic interventions is to secure sufficient caloric intake during periods of metabolic stress and fasting and to prevent the accumulation of β-oxidation intermediates. Therefore fasting should be avoided and adequate energy intake should be provided during infections and other fast-inducing conditions. In addition, a restriction of LCT (Long Chain Triglyceride) intake, supplementation with MCT (Medium Chain Triglyceride) and a high carbohydrate diet is recommended.[10, 19, 20] |
|
Niveau 4 |
The use of uncooked cornstarch at bedtime might be considered as a slowly released form of glucose.[8] |
|
Niveau 4 |
Carnitine supplementation is considered controversial, since the accumulation of long chain acylcarnitine esters possibly causes (ischemic) cardiac muscle pathology. At this time it is not known if the benefit of preventing carnitine deficiency outweighs this potential cardiac risk. Therefore oral carnitine administering is currently recommended only to prevent carnitine deficiency in plasma.[9, 20] |
|
Niveau 4 |
Essential long chain fatty acid supplements are recommended in case of deficiency. Low plasma concentrations of docosahexanoic acid (DHA) have been found in a few LCHADD patients and after supplementation improvement has been reported in vision and nerve conduction.[22, 23] |
Medicatie
Noodprotocol

Niveau 4 |
The main goal is to provide sufficient glucose to stimulate insulin secretion to levels that will not only suppress fatty acid oxidation in liver and muscle but also block adipose tissue lipolysis.[8] |
|
Niveau 4 |
In case of an acute episode the following measures are recommended:[24] ∙ Assess potential precipitants for metabolic decompensation (dehydration, fever, infection) ∙ If the patient is in a poor condition, check blood glucose, electrolytes, blood gas, ammonia and liver function tests. ∙ Check for cardiac symptoms (disturbed rhythm or cardiomyopathy) ECG, chest X-ray and/or cardiac echo. ∙ Start continuous glucose infusion in case of insufficient (oral) intake, vomiting, dehydration, hypoglycaemia, metabolic acidosis or decreased alertness. ∙ Correct hypoglycaemia and/or metabolic acidosis. ∙ Use intravenous glucose fluid with adequate electrolyte levels. ∙ Treat precipitating factors to minimize further catabolism. ∙ Increase glucose infusion if the patient fails to improve. |
|
Niveau 4 |
The following perioperative measures are recommended in LCHADD:[25] ∙ Stress-avoiding premedication should be given preoperatively. ∙ Thorough preoperative cardiac evaluation, including electro- and echocardiography. ∙ Glucose infusion (adapted to age and weight) should be given pre-, per- and postoperative. ∙ Glucose and CK should be monitored per- and postoperative. ∙ Use of propofol for short duration procedures might be considered[26]; use of succinylcholine and non-depolarizing muscle relaxants is controversial. |
